As a common malignancy with the highest incidence and mortality rates in the urinary system, urothelial carcinoma poses a substantial disease burden on society and patients' families due to its massive patient population and high propensity for recurrence and metastasis. In recent years, with the rapid development of tumor immunology and molecular targeted technologies, innovative therapies represented by PD-1/PD-L1 immune checkpoint inhibitors and antibody-drug conjugates (ADCs) are continuously emerging. These cutting-edge advances not only provide more armamentarium for clinical interventions in urothelial carcinoma but also profoundly reshape the treatment philosophy across the entire disease course.
Although authoritative domestic and international guidelines are updated frequently based on the latest evidence-based medicine, the real-world clinical practice in China still faces immense challenges in standardizing decision-making for complex cases and the evidence-based application, largely due to regional disparities in medical resources. Currently, frontline clinicians urgently need to clarify multiple core controversies and unmet needs. These include, for example: the optimal timing for immunotherapy intervention and bladder-preservation strategies in non-muscle-invasive bladder cancer (NMIBC) following Bacillus Calmette-Guérin (BCG) failure; the benefit assessment of perioperative immunotherapy in improving pathological complete response (pCR) rates in muscle-invasive bladder cancer (MIBC); the strategic sequencing of monotherapy versus "immunotherapy + ADC/chemotherapy" combination regimens and the popularization status of routine biomarker testing in the metastatic stage; as well as the real-world treatment patterns and accumulated clinical experience for upper tract urothelial carcinoma (UTUC) in the absence of exclusive high-level evidence.
To systematically address these clinical pain points and objectively map the real-world treatment landscape of urothelial carcinoma in China, the editorial board of the Bladder journal officially initiates the "2026 National Survey on the Real-World Clinical Practice of Urothelial Carcinoma in China" project. This project intends to utilize a scientific multi-stage stratified sampling design, covering tertiary and core secondary medical institutions nationwide, and will purposively invite over 1,000 key physicians from the departments of urology, medical oncology, and radiation oncology, who possess extensive experience in disease management, to participate in this in-depth survey.
This survey aims to objectively reflect the prescribing patterns and real-world practice concerns of frontline Chinese clinicians through the structured collection and multi-dimensional analysis of a large-sample, real-world dataset. The core findings of the survey will be compiled into the White Paper on the Current Landscape of Urothelial Carcinoma Diagnosis and Treatment in China and are planned for formal publication in the Bladder journal. Through the successful implementation of this project, we aspire to build a high-quality platform for academic exchange and consensus, providing solid data support to promote the standardized and precise development of clinical pathways for urological oncology in China, ultimately benefiting a broader patient population.
